Required to Check Out a Student Device
Parents and students, please complete the form at the link above for your students who will check out a laptop/Chromebook from the school. All students who borrow a laptop from the school will be required to have a completed form on file. Please provide a working phone number and email address on the form. Thank you.

From the Office of the Registrar:

Enrollment Packets, DDS Enrollment Forms for Driver’s Licenses, and any other documents requested by parents/guardians will only be processed for those students who have returned a completed, accurate Enrollment Student Information Form for the current school year. If we do not have a form for you, then we will not process any requests.
